About the position

Responsibilities
• Assess, plan, and implement individualized care strategies for members.

• Identify and initiate referrals for member transitions to community services and assist with social service programs.

• Manage the care plan throughout the continuum of care as a single point of contact.

• Communicate with stakeholders to ensure quality coordinated care and services are provided promptly.

• Advocate for members and families to ensure their needs and choices are represented and supported by the healthcare team.

• Visit Medicaid members in their homes and other settings, including community centers and hospitals.

Requirements
• Current, unrestricted RN license in the state of Texas.

• 2+ years of experience working within the community health setting or in a healthcare role.

• Intermediate level of computer proficiency, including Microsoft Outlook and Teams.

• Reliable transportation and ability to travel within the assigned region.

• Access to a designated quiet workspace at home with the ability to secure Protected Health Information (PHI).

• Reside in a location with access to a UnitedHealth Group approved high-speed internet connection.

Nice-to-haves
• 1+ years of experience with long-term care services and support, Medicaid or Medicare.

• Ability to create, edit, save, and send documents, spreadsheets, and emails.

• Knowledge of integrated settings principles, including federal and state requirements.
