Position: Remote Social Worker/Case Manager. Requires a 4-year degree in Psychology, Sociology or Human Services or Licensed Social Worker (LSW)
Locations: Remote Position. Travel required throughout Central Region of MN is required for meetings and visits with persons served. Based on the caseload, travel throughout other parts of Minnesota may be required.
Caseloads: Elderly/Alternative Care, Brain Injury, Intellectual Disabilities, CADI (Mental Health), and/or Managed Care Plans
Schedule: Monday - Friday (Full time) Typical daytime hours with occasional evenings as needed for meetings.
Wage: $49,000.00 - $51,600.00 annual salary based on qualifications. Wage will increase $2,000 after 6 months employment!
$51,600.00 annual salary for LSW (Licensed Social Workers). $49,000.00 annual salary for 4 year degree in Psychology/Human Services/Sociology.

Currently, our Case Management Department serves approximately 2,500 elderly, 1,500 people with mental health issues, and more than 1,000 persons with intellectual disabilities.

Job Summary:
As a Case Manager/Social Worker, you will be working on finding resources and services for persons (clients) that best fit their needs and situations. At Meridian Services, each Case Manager is an advocate for persons with disabilities/elderly and helps to make a difference in their lives. Our Case Managers will carry caseloads that consist of persons with intellectual disabilities, traumatic brain injuries, mental health diagnoses, and/or elderly persons.
In this position you will be completing the following:
Visits with persons on the caseload and attending team meetings
Completing or attend meetings on needs assessment; individual service plan development (CSSP)
Assisting with planning for new service development
Locating residential, vocational, and other needed services depending on each persons needs
Monitoring service delivery and ensuring the health and safety needs of each person are being met.
Completion of referrals and crisis intervention as needed
Developing waiver budgets
Completing case notes
Acting as an advocate for people we provide services to
Requirements: Have a 4-year degree in Human Services, Psychology or Sociology, OR be licensed as a Social Worker (Bachelor of Social Work required for licensure) or licensed as a Graduate Social Worker (Masters in Social Work required for licensure) as stated by the Minnesota Board of Social Work Experience working within the social services field A valid drivers license and daily access to reliable transportation with current insurance to travel throughout the Twin Cities and other parts of Minnesota based on caseload. Have an acceptable driving record as determined by Meridian Services policy. Successful clearance from a DHS background Study

Excellent computer skills including Microsoft office
Great organizational skills and attention to detail.
Knowledge of Medical Assistance, Medicare, and MN Health care programs
